Longevity medicine

Medicine for the years before disease announces itself.

Perennial is a physician-led longevity and concierge medical practice. We use advanced diagnostics, unhurried interpretation, and ongoing follow-up to understand where your health is headed and what is worth changing next.

Plain English

Longevity medicine is not “more tests.” It is better clinical timing.

Conventional medicine is very good when a problem has declared itself. Longevity medicine tries to see the trajectory earlier: cardiovascular risk before an event, muscle loss before frailty, metabolic dysfunction before diabetes, sleep disruption before it quietly drives blood pressure, appetite, mood, and recovery.

The Perennial model

What we actually do.

01

Build a baseline

We start with your history, goals, prior labs, medications, family risk, training, nutrition, sleep, symptoms, and a targeted measurement set.

02

Measure what changes decisions

Diagnostics may include labs, DEXA, VO₂ max, resting metabolic rate, sleep testing, cardiovascular risk assessment, selected genetics, or imaging when appropriate.

03

Interpret in context

Results are reviewed by Dr. Mottram in the context of your story, not treated as a generic dashboard score or an isolated abnormal value.

04

Adjust over time

The plan is revisited as your data, capacity, risk, and life change. Ongoing membership adds continuity, secure communication, and physician follow-up.

Different from usual care

The visit is not the product. The relationship is.

Common healthcare experience

Short visits, fragmented records, late thresholds, and advice that may not connect to your actual physiology.

Perennial experience

Unhurried physician visits, selected diagnostics, written priorities, and a plan that is adjusted as new information arrives.

Best fit

You want one accountable physician relationship.

Perennial is for proactive adults who want prevention, performance, metabolic health, aging strategy, and uncomplicated primary care interpreted in one place.

Not a fit

You want a single answer, not an ongoing plan.

If you only need one test result, one prescription, or the lowest-cost option, urgent care, a conventional primary-care visit, or a specialist referral may serve you faster.

Where to start

The first step is the Longevity Baseline Assessment.

The baseline is a paid clinical consultation and diagnostic review that produces a written plan. If membership makes sense afterward, the $695 baseline fee credits toward enrollment.
