The physician
Dr. Allan Mottram, MD FACEP
Perennial is designed around a direct physician relationship, not a disconnected menu of tests — and that relationship is with Dr. Mottram, who founded the practice and personally leads every patient's care.
Background
Dr. Mottram is a board-certified physician and Fellow of the American College of Emergency Physicians whose career spans emergency medicine, medical toxicology, and academic medicine. Emergency medicine teaches what actually fails in the human body — and how late most problems are caught. Toxicology adds a deep, mechanistic understanding of how medications and substances behave in individual patients. Longevity medicine is where he applies both: catching trajectory problems decades earlier, in patients who still have time to change them.

How he practices
Visits are unhurried — 90 minutes, not fifteen. Data from advanced diagnostics, wearables, and outside records is interpreted in the context of your history rather than scored against a generic template, and the plan is revisited continuously through the secure patient portal. The practice is intentionally small so that the physician reviewing your labs is the same one who knows your story.
