Perennial gives you one physician with time to go deep: longevity planning, primary care for uncomplicated patients, diagnostic interpretation, and the access level that matches your needs. Both memberships share the same physician and included core diagnostics. The difference is access.

How membership works

Perennial is direct-pay with transparent pricing: no insurance billing surprises inside the membership. Membership includes ongoing physician-led care, primary care for uncomplicated patients, structured follow-up, secure communication, plan review, and clinical interpretation of selected diagnostics. DEXA scan, VO2 max, and resting metabolic rate are included in both memberships. Labs and outside testing are billed separately. Some patients can use HSA or FSA funds for eligible services; confirm with your plan administrator.

What membership is not

Perennial is not an emergency service. Emergencies still require 911 or the emergency department. Patients with complex or specialist-managed conditions keep their existing care team, and Perennial coordinates alongside it. Membership terms, renewal, and cancellation are reviewed before enrollment so expectations are clear.
