Membership follows the clinical picture.
Not sure which fits? Start with a discovery meeting or the
Longevity Baseline Assessment. The right
level of access is a recommendation that comes after your goals, history, and first
measurement set are understood.
Perennial supports longitudinal longevity care, primary care for generally healthy adults,
and concierge access where appropriate. It is not an emergency service; emergencies still
require 911 or the emergency department. Membership terms, renewal, cancellation, and scope
of care are reviewed before enrollment so expectations are clear.
Based in Middleton, Wisconsin, Perennial cares for patients from multiple regions across
Wisconsin, several U.S. states, and the United Kingdom. Availability and scope of care
depend on location and individual circumstances. This page is informational and is not
individualized medical advice.
