Concierge medicine replaces volume with depth: a small practice, a direct relationship with your physician, 90-minute visits instead of fifteen, and care that continues between appointments instead of resetting at each one.
How membership works
Perennial is direct-pay with transparent pricing — no insurance billing surprises inside the membership. Membership includes ongoing physician-led care, structured follow-up, secure portal communication, plan review, and clinical interpretation of selected diagnostics. Outside labs, imaging, or third-party testing may be billed separately depending on the facility and your benefits. Some patients can use HSA or FSA funds for eligible services — confirm with your plan administrator.
Two levels of continuity
- Advanced Longevity — $6k/year. Ongoing physician-led care with advanced diagnostics and structured follow-up. The core membership for proactive, data-driven care.
- Perennial Concierge — $25k/year. The highest-touch tier, for patients who want direct year-round physician collaboration and concierge-level access.

What membership is not
Perennial supports longitudinal longevity care and concierge access, but it is not an emergency service — emergencies still require 911 or the emergency department — and it complements rather than replaces every primary-care need. Membership terms, renewal, and cancellation are reviewed before enrollment so expectations are clear.
