How membership works

Direct-pay care, interpreted by one physician.

Membership includes ongoing physician-led care, structured follow-up, secure portal communication, plan review, and clinical interpretation of selected diagnostics. Outside labs, imaging, and third-party testing may be billed separately. Eligible HSA or FSA use depends on your plan.

Care expectations

What membership is designed to do.

Perennial supports longitudinal longevity care, primary care for generally healthy adults, and concierge access where appropriate. It is not an emergency service; emergencies still require 911 or the emergency department. Membership terms, renewal, cancellation, and scope of care are reviewed before enrollment so expectations are clear.
