Why an assessment first

Most concierge practices ask you to join before you know what you're joining for. Perennial inverts that. The $695 assessment exists so the membership recommendation comes after a physician has actually looked at your picture — your history, your existing labs and records, your goals — not before. The full assessment fee credits toward membership if you continue.

What happens next

Some patients continue into membership; some take their baseline plan and return when the time is right. Either outcome is a success — the assessment is designed to be valuable on its own. Ongoing testing such as VO2 max and resting metabolic rate is included with membership. If additional diagnostics would change the plan, they are recommended specifically, with reasoning, never as a default bundle.

The Longevity Baseline Assessment is $695, direct-pay, with transparent receipts. The full $695 credits toward membership if you enroll. Some patients can use HSA or FSA funds for eligible medical services; confirm eligibility with your plan administrator or tax advisor.
