First step
The Longevity Baseline Assessment
A paid clinical on-ramp for new patients: physician interpretation of your data, a baseline plan, and a clear recommendation — before you commit to any ongoing membership.
Why an assessment first
Most concierge practices ask you to join before you know what you're joining for. Perennial inverts that. The $695 assessment exists so the membership recommendation comes after a physician has actually looked at your picture — your history, your existing labs and records, your goals — not before.
What it includes
- A new-patient clinical consultation with Dr. Mottram — unhurried appointments that generally run about 90 minutes.
- Physician review and interpretation of the data you bring: prior labs, imaging, wearable trends, and history.
- A baseline plan with clear priorities you can act on regardless of what you decide next.
- A direct recommendation on whether ongoing membership fits — and at which level — or whether it doesn't.
What happens next
Some patients continue into membership; some take their baseline plan and return when the time is right. Either outcome is a success — the assessment is designed to be valuable on its own. If additional diagnostics would change the plan, they are recommended specifically, with reasoning, never as a default bundle.
Pricing
The Longevity Baseline Assessment is $695, direct-pay, with transparent receipts. Some patients can use HSA or FSA funds for eligible medical services — confirm eligibility with your plan administrator or tax advisor.
The assessment is designed to complement your existing primary care. This page is informational and is not individualized medical advice.
