The first serious look at your health trajectory.

The Longevity Baseline Assessment is Perennial's new-patient starting point: an unhurried physician visit, review of your available data, a written baseline plan, and a recommendation about whether ongoing membership fits.

Why this comes first

You should not join a membership before the clinical picture is clear.

The baseline visit is designed to answer a practical question: what is worth measuring, what does it mean for you, and what should change next? Some patients continue into membership. Some leave with a plan and return later. Either outcome is acceptable.

What it includes

A physician visit that turns scattered data into priorities.

01

Prepare

Health history, goals, prior labs, medications, family risk, sleep, training, nutrition, and symptoms.

02

Review

An unhurried physician visit connects your data with your priorities and clinical context.

03

Measure

Diagnostics are recommended when they would change decisions, not as a default bundle.

04

Decide

You leave with a baseline plan, clear priorities, and a recommendation about membership fit.

What you leave with

A plan you can act on, even if you do not enroll.

Written priorities What matters most now, what can wait, and what should be tracked.
Testing rationale Which diagnostics would change the plan and why they are being recommended.
Membership recommendation Whether Advanced Longevity, Perennial Concierge, or no membership is the right next step.

If membership fits

The $695 credits toward enrollment.

If you continue into membership, the baseline fee is credited toward your first membership payment. Memberships are billed monthly.

Compare membership & access

If membership does not fit

You still have a usable plan.

The assessment is intentionally valuable on its own. You can act on the plan, share it with existing clinicians, or return when ongoing care makes sense.

How longevity medicine works

New patient booking

Start with the baseline, then decide what level of care makes sense.

This is the cleanest first step if you want clarity before committing to a membership.

Book the $695 Baseline Assessment

The baseline assessment is direct-pay. Some patients may be able to use HSA or FSA funds for eligible services; confirm with your plan administrator or tax advisor. This page is informational and is not individualized medical advice.
